Role Description The MDS Coordinator is a full-time, on-site role located in Snow Hill, NC, responsible for overseeing the Resident Assessment Instrument (RAI) process and ensuring accurate and timely completion of Minimum Data Set (MDS) assessments. This role collaborates closely with nursing, therapy, and interdisciplinary team members to gather clinical information, develop individualized care plans, and ensure documentation supports resident needs and regulatory compliance. The MDS Coordinator reviews clinical records, validates assessment data, and monitors schedules to meet federal and state requirements, as well as facility policies. Daily responsibilities include coordinating care plan meetings, updating care plans based on changes in condition, educating staff on MDS and documentation standards, and working with leadership to optimize quality measures and reimbursement. The role also supports quality improvement initiatives by analyzing MDS data, identifying trends, and recommending actions to improve resident outcomes.
Qualifications
- Current Registered Nurse (RN) license in good standing with strong geriatric nursing knowledge and experience.
- Demonstrated proficiency in MDS assessment processes, RAI guidelines, and related regulatory requirements.
- Experience in care planning, including developing, implementing, and updating individualized care plans.
- Strong clinical assessment, documentation, and critical-thinking skills, with attention to detail and accuracy.
- Effective communication and collaboration skills for working with interdisciplinary teams, residents, and families.
- Familiarity with long-term care and skilled nursing regulations, quality measures, and reimbursement systems.
- Proficiency with electronic health records and related clinical software; strong organizational and time management abilities.
- Preferred qualifications: Experience as an MDS Coordinator in a skilled nursing facility, RAC-CT or similar MDS certification, and knowledge of Medicare and Medicaid documentation standards.
