Role: Home Care Travel Registered Nurse
Responsibilities:
- * Deliver high-quality, person-centered care in patients' homes across assigned regions.
- Perform comprehensive assessments and develop or update individualized care plans.
- Administer medications and treatments, monitor both chronic and acute conditions.
- Collaborate with physicians, behavioral health specialists, and interdisciplinary teams to coordinate services and ensure evidence-based care.
- Accurately document all care activities in electronic health records.
- Educate patients and families on health management and care procedures.
- Maintain regulatory compliance and adapt to changing caseloads and travel schedules.
- Uphold values of integrity and accountability in all aspects of work.
Qualifications & Skills:
- * Registered Nurse (RN) licensure.
