Key Responsibilities
- Welcome and orient patients and families.
- Facilitate weekly patient and parent groups.
- Provide individual follow-up and support.
- Support families with relapse-prevention tools.
- Collaborate with therapists and the multidisciplinary team.
- Monitor attendance, engagement, and progress.
- Maintain documentation and prepare reports.
Qualifications
- Degree in Social Work or a related field.
- 10 years of experience in social work.
- Experience in addiction recovery is a strong advantage.
- Strong communication and group facilitation skills.
