Brief description of duties:
- Conduct psychosocial assessments and identify beneficiaries' needs and vulnerabilities.
- Provide case management services, including follow-up and documentation.
- Deliver psychosocial support and supportive counselling to individuals and families.
- Facilitate referrals and coordinate with health, mental health, protection, and social service providers.
- Support community outreach, awareness-raising, and psychoeducation activities.
- Maintain confidential records and ensure timely reporting.
- Participate in multidisciplinary team meetings and case conferences.
- Support the implementation of Mental Health and Psychosocial Support (MHPSS) activities in line with MoPH and NMHP guidelines.
- Contribute to strengthening community-based mental health services and social support networks.
- Monitor beneficiaries' progress and ensure continuity of care through regular follow-up.
Education:
- BA or MA in Social Work
- Experience in PHCC is a plus
- Experience: 1-3 years
